- Entry Physicals - Please be advised that all students who are new the school district must provide evidence of a recent physical examination ( within last 365) to the Nurse's Office. Receipt of this physical exam is required with 30 days of admission to school in order to meet this requirement. If your child has not had a recent physical examination and you will be scheduling an appointment with your primary care practitioner, please call our office (404-2025) with appointment date. If you have any question or concerns regarding this or any other health issue, please feel free to contact the Nurse's Office during normal school hours.
- Sports Physicals - Those who wish to participate in sports must have, a physical examination by their private - physician. The examination of a student for a sports team shall be conducted within 365 days prior to the first day of practice for that sport.
